Equitable Health
Journeys for All People

We don’t believe a person’s health story should be written by their income, ZIP code and race. Yet, all too often, this is the case. The PatientPoint Foundation was created to bring a different approach to health equity issues and help mitigate health disparities.

We are taking the basic principles of community work — proximity, relationship, and trust — and pursuing modern, cutting-edge solutions with best-in-class data analytics to change the dynamic in community health.

Join us in reaching those in high-priority communities as we put empowering information and equitable health journeys within their grasp.

Our Model for Community Impact

Connectivity + Community

The PatientPoint Foundation’s unique and integrated approach will provide a new model in which we empower people and impact lives by:

  • Leveraging the voice of the community
  • Breaking down barriers as they relate to health
  • Connecting high-priority communities to proven education, and engaging technology and content
  • Empowering people to be more “health engaged” and adopt healthier behaviors
  • Utilizing best-in-class data analytics and outcomes analyses

Addressing Root Causes of Health Disparities

Present inequities across our health system limit under-served people’s access to affordable, high-quality care, create avoidable costs and can be clearly linked to negative patient outcomes. According to one Deloitte analysis, health disparities can cost the economy roughly $42 billion in lost productivity per year.”

Health Equity exists when all people have access to opportunities to thrive, both physically and mentally, and no one is limited in achieving health and wellness because of their race, ethnicity, nationality, gender, ability, sexual orientation, age, income, or zip code.

This is achieved when every person can attain their full potential for health and well-being.

The mission of the Foundation is to help more priority communities achieve health equity. We seek to use the tools and assets at our disposal to kickstart positive health journeys. We’ve set out to make EVERY person’s health journey better so people can live longer, healthier lives.

The Work We Do

Community + Connectivity

The Foundation is actively involved in high-priority communities where the data on health metrics and patient outcomes shows a clear disparity. With the full support of our partners, we introduce proven technology and health education into those communities in novel ways.

For us, it’s about the right message in the right place at the right time. Thinking outside the healthcare box, we start in the community by providing unique access and engagement where one does not always expect to find it.
